[WIP] libvlr: proper handling of the MNRF and sending of readyForSM

This is an early, non-working attempt to implement proper handing
of the MNRF (MS Not Reachable Flag) and sending of the readyForSM
primitive (reason=MS_PRESENT) to the SMSC.
